To get to these settings go to setting>general>about


-Device type
-Firmware Number(version)
-Modem firmware or baseband


What Jailbreak tool did you use?

What error numbers did you get in Itunes?(If applicable)

What have you already tried To fix the Problem?

For cydia app crashes try to include the app version
